NSW Planning Management is here to help you manage your NDIS budget. We are responsible for the financial aspects of your plan, including contacting NDIA, paying providers and vendors, processing fees and monthly reports on all your expenses. Support Coordination
We can oversee your NDIS plan by coordinating services and trainings required for greater experience and improve your skills and abilities. This includes helping you choose the right service, and ensuring your goals are ultimately met. We provide short-term or ongoing support to coordinate support services, live independently and develop your ability to participate in community life. And helping you to manage life stages, transitions and support services.

Respite Care
Caring for a loved one 24 hours a day can be physically and mentally challenging. It difficult to keep up with your own work while you focus on providing the best care possible. To avoid this, even the dedicated caregivers need to take a break every now and then. If you take out the time to take care of yourself, you can take care of your loved ones. Whether you need a short break to recharge, require regular support, or a short postponed vacation, respite care is a great option.

Support Coordination

If you are a NDIS Participant and your NDIS plan has approved the "Coordination of supports" then you will be able to receive the following assistance : helping you to coordinate or manage life stages, transitions and support services, we provide support to coordinate support services, live independently and develop your ability to participate in community life.

Respite Care
Caring for a loved one 24 hours a day can be physically and mentally challenging. It will also be difficult to keep up with your own work and commitments while you focus on providing the best care possible. Our respite care professionals strive to build on trust and ensure respect and service is delivered in highest standard. Our Respite home services are flexible and not limited to staying at home. Our nurses will be happy to accompany and support your loved one on special trips and activities. Our friendly support carers will help you to vacuum, clean floors, toilets, bathrooms, showers.

Community Care Participation
As we get older, most people would like to stay at their own homes. Similarly it applies to persons with disabilities. They will feel safe and comfortable in a familiar environment. However, there is downside to not being in constant contact with your local community and your close family friends. We understand the importance of community support and regular interactions with local community. We will choose a friendly carer for you based on your needs and preferences, so that you are well taken care of. Enjoy visits and participations on days and times that suit you.

Transport
Even if you don't drive anymore, there are still places to need to visit, people to see and daily life activities to do. We understand how important it is to attend appointments and activities and continue to live as independently as possible. And with the help of our friendly team, you will arrive on time for all your activities and appointments like visiting banks, going out for shopping, attending social activities and recreations, appointment of doctors , visiting family, friends and community events. Our transport staff are more than just drivers. They are well thought out and well trained.

Assistance with Social and Community Participation
This support is included in the participant's plan so the participant can engage in community activities related to participant’s disabilities and goals you want to achieve. These activities can be carried out within the centre or community, and these activities cost may vary as standard or intensive rates. The NDIS funding does not cover some costs associated with recreational activities, such as tickets to events for participants and paid support staff for social or recreational activities. Our staff will guide you through the process and help you participate in community, social, and club activities in your area.

Personal Care
We at NSW Plan Management provide a tailored service to meet the needs of our clients and coordinate with other services to enable them to live independently in their own homes with the necessary support. NSW Plan Management, the services we provide are based on individual needs, enabling people to provide 24-hour care when they need it, enabling them to go about their daily lives. Regardless of patient's care needs, we will provide the necessary level of care at home.

Mental Health Services
We are a NDIS Registered Provider in mental health service and is known for being compassionate and empathetic in industry. Becoming Independent or recovering from injury means very different to everyone. Improvements include managing daily activities, participating in social activities, being able to live independently, gaining self-confidence, and dealing with life changes such as returning to work or education. We are here for you. Support is available to overcome difficulties, stress and fear in your life.

Behavioural Support
What does positive behavioural support means? Positive Behavior Support (PBS) is the most effective and authentic way to manage and support complex behaviors. Positive behavior support can be used to achieve behavior change in children and adults. The goal of PBS is to reduce the incidence of complex and challenging behaviors while developing the skills needed to be independent of those behaviors in the future. Not only behavior, it's about building skills, learning to communicate, working to manage emotions, etc.
